Vampire Diaries fans – are you ready to feel feelings?

Steven R. McQueen‘s final episode on The Vampire Diaries airs Feb. 12, and PEOPLE has your exclusive first look at Jeremy’s last day in Mystic Falls before he leaves town for art school.

In the photo, we see Jeremy smiling, with his bags packed, while talking to his de facto father figure, Alaric (Matt Davis). It’s fine – we didn’t need our hearts, anyway.

The episode, “Stay,” will have Elena (Nina Dobrev) putting on a brave face as she reminisces with her brother about their past and their time spent in Mystic Falls. Meanwhile, Stefan (Paul Wesley) and Caroline (Candice Accola) grow ever closer as the two prepare the Forbes family cabin so that Caroline’s mother, Sheriff Forbes (Marguerite MacIntyre), can live out her final days in comfort.

Later, a confrontation with Enzo (Michael Malarkey) leads Jeremy to question his decision to leave Mystic Falls after all. The sure-to-be-heartbreaking episode (what else is new for TVD?) will also see Caroline rushing to her mother’s side after finding out she’s taken a turn for the worse.

McQueen is leaving after having been a part of the show since the very beginning. Prepare your tissues.

The Vampire Diaries airs Thursdays (8 p.m. ET) on The CW.
