Luke Perry Was Buried in Tennessee, Death Certificate Reveals

The Beverly Hills, 90210 star died March 4, five days after suffering a massive stroke

Luke Perry‘s final resting place has been revealed.

According to a copy of his death certificate obtained by PEOPLE, Perry was buried in Dickson, Tennessee on Monday. The actor was a part-time resident of Tennessee and had owned a farm there since 1995, according to TMZ, which first reported the news of the death certificate.

The Beverly Hills, 90210 star died last week after suffering a massive stroke. The death certificate lists the cause of death as an “ischemic cerebrovascular accident,” also known as a stroke.

Perry was hospitalized on Feb. 27 after paramedics were dispatched to his home in Sherman Oaks, California. The actor remained “under observation” for five days but died on March 4. According to the death certificate, he died at 12:44 p.m. He was 52.

“He never regained consciousness,” a source told PEOPLE. “Things were so dire that they put him under heavy sedation. He was gone, basically, by the time he got to the hospital.”

Perry’s fiancée Wendy Madison Bauer is listed as his next of kin, or “informant,” on the certificate. According to a “save the date” notice obtained by TMZ, Perry and Bauer were planning to tie the knot on August 17, 2019.

Last week, Bauer spoke out for the first time since Perry’s sudden death.

“I want to express my gratitude to everyone for the outpouring of love and support,” she said in an exclusive statement to PEOPLE. “The countless, heartwarming stories of Luke’s generosity and kindness have been a great source of solace during this difficult time.”

“The past 11½ years with Luke were the happiest years of my life, and I am grateful to have had that time with him,” she continued. “I also want to thank his children, family, and friends for their love and support. We have found comfort in one another and in the knowledge that our lives were touched by an extraordinary man. He will be dearly missed.”

Perry’s rep announced the actor’s death in a statement, confirming he was “surrounded by his children Jack and Sophie, fiancée Wendy Madison Bauer, ex-wife Minnie Sharp, mother Ann Bennett, step-father Steve Bennett, brother Tom Perry, sister Amy Coder, and other close family and friends.”

In lieu of flowers, the Perry family asks that donations be made to Fight Colorectal Cancer and St. Jude’s Children’s Hospital.
