Rob Kardashian‘s personal struggles don’t seem to be improving, despite continued efforts from his famous family.

In a new sneak peek for Sunday’s episode of Keeping Up With the Kardashians, Kris arranges for a life coach to meet with her, Kourtney and Kim (despite Kim’s protestations) to devise a plan to help their struggling sibling.

In the clip, Kris admits to her daughters that she feels like “If I don’t help him do something drastic, that [Rob]’s gonna die.”

Rob has made headlines over the past year over his extreme weight gain and alleged issues with depression.

“The most important thing in my life to me, is my children,” says Kris. “This is somebody who can help dissect the situation and figure out exactly what’s going on.”

Kris explains the change in her son to the life coach, mentioning his turn on Dancing With the Stars and previous athleticism. “I mean he obviously has some kind of depression,” adds Kim.

The Jenner matriarch breaks down into tears, admitting that she feels like she’s “waiting for this horrible thing to happen,” and there’s no preventing it.

Keeping Up with the Kardashians airs Sundays (9 p.m. ET) on E!
