In an exclusive sneak peek of TLC’s hit reality show 19 Kids & Counting, airing Tuesday at 9:30 p.m., Jill and Derick Dillard go to their very first midwife appointment – and hear their baby boy’s heartbeat for the first time. Last week’s season premiere ranked as the No. 1 cable program with women on Tuesday night, according to a press release from TLC. This week will feature two episodes back-to-back: First, fans will get to watch Jill’s younger sister Jessa Duggar, 22, go shopping for wedding dresses; then Jill and Derick give an on-the-scene look at a meeting with their midwifery team. Since revealing her pregnancy exclusively to PEOPLE, Jill, 23, and Derick, 25, have been early anticipating their child’s birth. “I have been studying midwifery for three and a half years,” Jill says in the clip. “I have a lot of experience in that area, but it is entirely different when you are the one pregnant.” Derick, who has just one sibling, marveled at hearing his child’s heartbeat. “When I first heard our baby’s heartbeat, I have never heard a heartbeat in utero, it is extra special for me,” he says in the clip. As the couple prepares for a home birth at their Rogers, Arkansas, residence, they tell PEOPLE they’re logging Jill’s protein intake and trying to exercise together. “We went swimming last night and yesterday,” Derick says, “and we are logging Jill’s protein, trying to stay on track recording it.” Jill reports that she’s still craving spicy food. “I was just eating egg casserole with a lot of hot sauce on it,” she says. “I’m eating a lot of spicy stuff these days.” For more on Jill and Derick’s birth plans, and an at home interview with Jessa and Ben Seewald, pick up the current issue of PEOPLE on newsstands now
